In completing this application I am certifying that I am not a user of drugs, drink alcoholic beverages in excess, and have not been convicted of a felony. Further, I state that I am not, at this time, or within the past three years, been under psychiatric care, the nature of which would effect my relationship with Little League children.

Any Manager and/or Coach that will have direct and sole supervision over players must have their background check completed before working with their teams. Our coaching coordinator will have an approved list of those coaches that can begin on field work with their teams.
